Registration for FIVE LENS Certification Training

Practitioner Level & Team Facilitator Level Certification

Please note requirements to attend FIVE LENS Practitioner Training:

  1. You need to have (or be in the process of acquiring) training and experience in individual coaching, counselling, mentoring, or related personal development skills.

 

  1. The FIVE LENS assessment is designed to be used in personal development contexts and NOT in recruitment & selection. By attending this training you agree NOT to use FIVE LENS assessments for recruitment & selection purposes.

 

  • After attending the facilitated group training you will be required to complete two case studies for which you will enrol two clients to complete the FIVE LENS Questionnaire, debrief them individually through their Personal Feedback Reports (PFRs), complete feedback forms on your experience and request feedback from your two clients. We will guide and support you through this process. This will comprise your final evaluation for certification. The cost of the two case study PFRs is included in your course fee.

 

Please note requirements to attend FIVE LENS Team Facilitator Training:

  1. You must have successfully attained certification at FIVE LENS Practitioner Level.

 

  1. You must have experience (and preferably been trained) in the facilitation of groups or teams in a learning context. Please indicate your facilitation experience in the space provided in this registration form.

 

  1. On successful completion of our FIVE LENS Team Facilitator Certification you will have future access to the on-line learning program and you will be able to contact us for any individual support that you may need.

 

  1. After completing the online training you will be required to facilitate a FIVE LENS Team Discovery Day (TDD) or Team Performance Process (TPP) and complete a Feedback Form reviewing your experience of the process. We will also require Feedback Forms from the participants.

 

  1. PLEASE NOTE: The cost of the course includes a Lifetime access to the Course Materials and on-going Support & Preparation Calls with our Certification Moderator. The cost of products needed to run your first TDD (case study for Certification) are not included and need to be purchased. (Team member numbers may vary, and so we don’t provide promotional credits for the Team Facilitator Course. This helps us to keep costs low.)

  • 2 x sets of Team Stage Cards – R170.00 (excl.VAT) SA only
  • PostNet2PostNet Domestic Courier (residing in South Africa) for the Adjective & Team Stage Cards – R109.00 (incl.VAT) SA only

If you are outside of South Africa or as a second option for South Africans, the PDF documents for the cards are also available for free for you to download, print and laminate yourself. We will provide the Google Drive link for this.
